Overview

As a Virtual Care Referral Coordinator, you will play a pivotal role in orchestrating various referral orders. This includes managing both internal and external referrals for specialty care and imaging services. Your work directly contributes to our commitment to providing every member with exceptional healthcare experiences, connecting them with the right providers and ensuring smooth transitions within their care journey.

Key Responsibilities

Act as a vital part of our virtual primary care team, assisting members through phone calls and messaging platforms.
Ensure all external and internal referrals and orders, such as specialist referrals, imaging orders, and prior authorizations, are completed accurately and routed efficiently to the correct external providers.
Support patients by providing essential information, including maps, facility locations, and direct dial numbers, making their appointments easier to navigate.
Coordinate the collection and transfer of medical records, ensuring all necessary documentation is available for a seamless care experience.
Schedule appointments and perform insurance verifications to facilitate member access to required services.
Proactively request results or additional medical records as needed to support ongoing care.
Communicate timely updates to both providers and members, keeping everyone informed throughout the referral process.
Go above and beyond to identify and secure the highest quality providers for our members.
