Overview

Working alongside the Manager of Care Management, you will act as a trusted healthcare guide and advocate. Your daily conversations will help uncover unmet needs, clarify wellness goals, and introduce members to essential clinical and community resources. Through targeted evaluations and empathetic outreach, you will empower patients to take active control of their health.

Key Responsibilities

Your work centers around human connection, careful evaluation, and proactive coordination. Everyday tasks include

Conducting comprehensive assessments over the telephone to understand individual health conditions and daily barriers
Designing and guiding personalized care plans focused on measurable health improvements
Identifying gaps in care and coordinating solutions for issues like medication access, specialist appointments, and transportation
Tracking member progress continuously through clinical documentation, data review, and regular follow-up calls
Working in tandem with multidisciplinary care teams to deliver a unified support network for each patient
