Position Overview
We are seeking dedicated Licensed Practical Nurses to join our team as Care Navigators. This is a fully remote role open to candidates worldwide. As an LPN Care Navigator, you will serve as a vital link between patients and their care teams, guiding individuals through their chronic care management journeys with warmth and professional expertise.
Key Responsibilities
- Engage proactively with patients via phone and digital channels to monitor chronic conditions.
- Educate patients on healthy lifestyle choices and adherence to their prescribed care plans.
- Review biometric data and coordinate follow up actions with physicians when readings fall outside target ranges.
- Maintain accurate and timely electronic health record documentation for every patient interaction.
- Lead every conversation with empathy and ensure patients feel supported at all times.
Requirements
- Current and unencumbered LPN license in your state or region.
- Prior experience in clinical nursing, chronic care management, or telehealth is a strong plus.
- Exceptional communication skills and a deeply compassionate bedside manner.
- Comfort with remote work technology and digital health platforms.
