As a Remote Hospice Triage LPN, you will become an essential extension of our clients’ care teams, supporting patients globally during their non-core hours. This is a full-time, fully remote position offering a consistent schedule from Monday through Friday, 6:30 AM to 4:00 PM, with no weekends required.

Key Responsibilities

Provide compassionate and professional telephone-based triage for hospice patients and their families.
Accurately assess patient symptoms, concerns, and needs over the phone.
Utilize customized client protocols to guide clinical decision making and care coordination.
Ensure timely and appropriate interventions, facilitating communication with on-call field nursing staff as needed.
Document all patient interactions meticulously and efficiently in electronic systems.
Offer empathetic support and education to families navigating challenging situations.

Requirements

Possess a valid, active Licensed Practical Nurse (LPN) license.
Demonstrate exceptional empathy, professionalism, and strong interpersonal communication skills.
Previous experience in hospice, home health, or a related clinical setting is highly preferred.
Must be comfortable and proficient with computer systems and electronic health records.
Ability to work independently and effectively in a remote home office environment.
A reliable internet connection and a quiet, dedicated workspace are essential.
Commitment to a consistent full-time schedule, Monday through Friday, 6:30 AM to 4:00 PM (specific time zone details will be discussed during the interview process).
