Navigating mental and behavioral healthcare can feel completely overwhelming for individuals and families facing complex challenges. At Humana, we believe that everyone deserves thoughtful, dedicated guidance to bridge the gap between their daily struggles and the specialized clinical care they need. We are looking for an empathetic Field Care Manager in Behavioral Health to champion our members, listen closely to their stories, and create sustainable paths toward wellness. This position represents a fully remote, worldwide opportunity, enabling you to bring your clinical heart and coordination expertise to our community from wherever you live.
Role Overview
In this role, you serve as the central anchor for members across diverse stages of life, ranging from young children and adolescents to adults navigating serious mental illness. You will conduct thorough health assessments, identify social determinants that impede recovery, and coordinate with clinical providers to deliver timely interventions. Your everyday focus centers on treating the entire person, ensuring care plans respect unique lived experiences while closing critical gaps in behavioral healthcare.
Key Responsibilities
Your day will involve a dynamic mix of direct outreach, collaborative clinical planning, and creative problem solving. Some of your primary duties include the following tasks.
- Conduct comprehensive behavioral health evaluations and telephonic assessments to uncover clinical, social, and emotional needs.
- Act as the primary point of contact for members, guiding them toward appropriate behavioral health and medical providers.
- Deliver evidence-based crisis intervention support and practical de-escalation strategies during urgent circumstances.
- Recognize environmental and systemic barriers to effective treatment, stepping in proactively to resolve them.
- Collaborate with multidisciplinary healthcare networks, peer support specialists, and community agencies to build integrated care plans.
- Track member progress over time, updating care strategies to ensure lasting behavioral health outcomes.
Requirements
We are searching for a dedicated professional who pairs sound clinical judgment with authentic human empathy. The ideal background includes several key qualifications.
- Professional experience in behavioral health, social work, psychology, nursing, or a closely related human services field.
- Proven background supporting vulnerable populations, including adults with serious mental illness as well as children and adolescents.
- Strong crisis management capabilities and the confidence to guide members through acute distress.
- Familiarity with evidence-based behavioral health practices, care coordination models, and integrated medical approaches.
- Excellent communication skills and the self-motivation required to thrive in a global remote setting.
What You Will Gain
Joining Humana means stepping into an organization that honors your commitment to healing and advocacy. You will work alongside passionate professionals who share your dedication to health equity and person-centered care. Beyond expanding your clinical coordination expertise, you will experience the genuine fulfillment that comes from helping individuals reclaim stability in their lives, all while enjoying the freedom and flexibility of a supportive remote workplace.
