Overview

As our Director Patient Care Services, you will provide critical leadership and strategic direction for Hoag's home health clinical services. This pivotal role involves setting, implementing, and evaluating clinical goals and objectives that champion superior patient care, ensure strict regulatory compliance, and perfectly align with our organization's mission and philosophy. You will also serve as the dedicated Clinical Manager, fully meeting Medicare Conditions of Participation, while offering comprehensive clinical oversight and operational guidance to guarantee the safe, effective, and compliant delivery of all home health services, adhering to local, state, federal, and accreditation standards.

Key Responsibilities

Provide overall leadership and strategic direction for all home health clinical services.
Establish, implement, and continuously evaluate clinical goals and objectives, ensuring they uphold quality patient care and regulatory compliance.
Serve as the Clinical Manager, meticulously adhering to all Medicare Conditions of Participation.
Offer robust clinical oversight and operational guidance to the home health team.
