Overview

This is a full time, regular position, offering 40 scheduled weekly hours during daytime shifts. As a Certified Community Health Worker (CCHW) within our Social Needs Resource Hub, you will play a crucial role, working remotely from anywhere in the world to support patients. Your focus will be on addressing social determinants of health, connecting individuals to vital resources. While Geisinger’s primary service area is within the United States, this telephonic engagement role offers global remote flexibility, making a significant impact on health equity.

Key Responsibilities

Conduct patient centered outreach, primarily through telephonic engagement, for individuals with identified social needs.
Collaborate actively within a team based model alongside healthcare teams, community based organizations, and hub colleagues to assess patient needs and coordinate appropriate support.
Utilize motivational interviewing and effective problem solving techniques to educate patients and connect them with community resources. These resources address critical social determinants of health such as transportation, food insecurity, housing, financial strain, and utilities.
Facilitate timely referrals and partner strategically with community based organizations through the Neighborly platform to ensure successful linkage to services.
Perform follow up outreach to confirm successful connections and proactively address any barriers patients may encounter, contributing to a shared team workflow and accountability model.
Maintain accurate and timely documentation within Geisinger’s electronic medical records system, ensuring comprehensive patient records.

Requirements

A current and valid certification as a Community Health Worker (CCHW) is essential for this role.
Strong communication and interpersonal skills, especially for telephonic engagement, are critical.
Demonstrated empathy and a genuine desire to assist individuals facing social challenges.
Ability to work effectively in a remote, team based environment.
Proficiency in utilizing various communication and documentation platforms.
