Key Responsibilities
- Engage with members to deeply understand their specific behavioral health needs, challenges, and personal goals.
- Develop, implement, and monitor personalized care plans in collaboration with members, their families, and other healthcare providers.
- Act as a primary point of contact, providing guidance, resources, and support to members throughout their care journey.
- Identify and actively work to remove barriers to care, ensuring members have access to necessary services and support systems.
- Empower members with tools and knowledge to take an active role in managing their behavioral health and achieving desired outcomes.
- Collaborate closely with a multidisciplinary team of clinicians, primary care providers, and specialists to ensure coordinated, integrated care.
- Maintain thorough, accurate, and confidential documentation of all member interactions and care activities in compliance with regulatory standards.
- Advocate on behalf of members to ensure their voices are heard and their care needs are met effectively.
Requirements
- A strong clinical background, likely including a master's degree in a relevant behavioral health field (e.g., Social Work, Counseling, Nursing) and active, unrestricted licensure.
- Demonstrated experience in behavioral health case management or care coordination.
- Profound understanding of various behavioral health conditions, treatment modalities, and community resources.
- Exceptional communication, interpersonal, and active listening skills, fostering trust and rapport with diverse populations.
- Strong problem solving abilities and the capacity to navigate complex healthcare systems.
