This is a fully remote, worldwide opportunity that allows you to make a tangible difference in behavioral health and community well-being.

Key Responsibilities

You will develop, assess, and facilitate complex care management activities tailored primarily to mental and behavioral health needs. Your day-to-day involves creating personalized care plans, providing vital education to members and their families regarding substance use disorders, and evaluating resource availability through phone calls and home visits.

Requirements

Candidates should be ready for a Monday through Friday schedule operating mainly from 8:00 a.m. to 5:00 p.m., with occasional evening flexibility when family needs arise. While the role is remote, it is field-based in northwest Kansas with a preference for applicants residing in or near Ellis, Russell, Trego, or Rooks counties, as you will travel up to 75 percent of the time throughout your assigned region.
