WebTPA, a proud GuideWell Company connected to the Florida Blue family, stands as a healthcare third-party administrator with an impressive three decades of experience crafting unique benefit solutions and expertly managing customized health plans. In this essential Care Coordinator role, you'll be instrumental in connecting the dots between administration, Care Managers, patients, and their family members. Your daily efforts will help individuals achieve their healthcare goals while ensuring clear, consistent communication. This is a crucial non-clinical support position, designed to provide additional layers of assistance to our dedicated clinicians, case managers, and valued members as they navigate their patient care journeys.

Key Responsibilities

You will serve as a primary resource and educational contact for patients, their families, healthcare providers, and our internal staff, providing valuable information and guidance.
Engage in telephonic follow-up conversations with members, offering crucial support particularly after hospital discharge and during ongoing chronic condition management.
Diligently complete all assigned tasks related to comprehensive care coordination, ensuring every detail is handled with precision.
Generate and assign necessary tasks to the designated RN Case Manager, facilitating seamless teamwork and patient progress.
Direct patients and their families to the most appropriate internal and external resources, including valuable community programs and services.
Under the direction and supervision of an RN Case Manager, you will assist members in achieving their personal health goals. This includes offering unwavering support, providing relevant member education, facilitating access to essential resources, and arranging follow-up care as needed.
Maintain accurate and up-to-date records of all interactions and progress, ensuring thorough documentation.

Requirements
